Obtaining Letters of Recommendation
from Justice Center or Other UAA Faculty

Students frequently request letters of recommendation from faculty members for financial aid, job applications, applications to graduate or law schools, or other purposes. The forms below, in Adobe Acrobat .pdf format, can be filled out online and printed out (but cannot be saved online).

For each faculty member from whom a recommendation is sought, both a Request for Letter of Recommendation form and a signed, dated Student Reference Request, FERPA Release, and Release of Liability form is required. Be sure to read and follow all instructions. Both forms require additional information such as a resumé, information about career goals, and addresses of organizations for which you are requesting a recommendation. Submit completed forms (including additional information) to the faculty member's academic department.
